For remote care, monitoring is crucial. Clinicards has the technology to easily deploy telemonitoring. The flexibility of Clinicards allows for countless combinations for any disease condition. Patients keep track of their data in the app on the phone, and healthcare providers get this data in the Clinicards Portal, or integrated into the EHR.

Often uncertainty plays a big role in patients. What was the diagnosis again? What’s in store for me, and when? The right information at the right time is crucial in this regard. No more paper leaflets, but handy information on your mobile. ContentCards and CarePath within our Clinicards platform allows the healthcare provider to specify what information should be shown on the patient’s app and when. Adding content yourself is easy through the CMS, but hooking into already existing content from other (academic) hospitals is even easier.

The Clinicards platform comes into its own when coaching patients toward better health. Parent and child
